Preschool of the Arts Registration Process

Fill in the Child Care Request Form online

You will be contacted by email or by phone when your request form is processed and arrangements can be made for a tour of the preschool. Once we have arranged a tour, answered questions and confirmed a spot, we will require the following forms to be completed and processed: Preschool of the Arts Registration form, Immunization form and "Getting to know your child" form. A registration fee of $100 is due with the completed forms. We only except cash, cheque (payable to Preschool of the Arts Forest City Ltd.) or e-transfer is preferred.

Family Tree Music Program Registration Process

If you would like to register for one of our Family Tree Music classes, please fill out the form below and let us know what days and times you would like for your child. Please note that registration is not complete until your registration form is processed, and payment arrangements have been made. We only except cash, cheque (payable to Preschool of the Arts Forest City Ltd.) or e-transfer.
